Application for Unit Standard Assessment

An application for assessment should be made when the trainee and the training coordinator are satisfied that the trainee has had sufficient training and practical experience to be able to confidently demonstrate the knowledge and skills defined in the unit standard.

The trainee shall complete an Application for Assessment (Form F07) and forward it to Opportunity Training. (Refer to flowchart "Process for Assessment")

The application must include:

  • Training agreement details including agreement number.
  • NZQA registration number where this is available.
  • A list of the unit standards for which assessments are requested.
  • A list of related unit standards already attained, including any required prerequisites.

Unit Standard Assessments

On receipt of the Application for Assessment (Form F07) the assessor shall make contact with the trainee and training provider to arrange for a time, place and the necessary equipment for the assessment. A copy of the completed Assessment Plan Form (Form F08) will be forwarded to the training coordinator.

The assessment of competency to the unit standards shall be based on evidence. Evidence must be sufficient and may be presented in a variety of forms including:

  • specific training completed.
  • prior learning including certificates, awards etc.
  • experience including log books, references and letters from past employers etc.
  • demonstration of skills in completing specific tasks.
  • ability to answer verbal and/or written questions.

Unit standards require that tasks be assessed against equipment manufacturer's requirements, generally accepted industry standards and the enterprise procedures. The enterprise procedures shall be those applying at the workplace or private training establishment where the training has been based.

The enterprise procedures shall be made available to the assessor prior to the assessment to enable an assessment plan to be developed. Where documented procedures are not available the training coordinator shall ensure that a person who is fully conversant with the accepted procedures is available during the assessment.

Documented procedures normally available to a trainee during the course of operations shall be made available during assessments. Reference may be made to these during assessments, however constant referral may not demonstrate confident ability.

The Judgement Guide supplied to the trainee for reference during training shall not be available for reference during the assessment.

The assessor shall arrange with the training coordinator for the necessary equipment to be made available at the time of the assessment.

The trainee shall complete the assessment requirements as directed by the assessor.

The assessor shall record the assessment results and comments where appropriate.

Outcome of the assessment will be one of the following:

  • Trainee is competent.
  • Trainee is not yet competent.
  • Additional evidence required.

Feedback shall be given to the trainee at the conclusion of the assessment process. If the assessor requires more evidence that is not immediately available, then the assessor shall explain what is required and how the evidence may be presented to satisfy the requirements.

An Assessment Report, (Form F09), shall be completed by the assessor. A copy shall be given to the trainee, a copy retained by the assessor and a copy sent to Opportunity Training complete with other essential supporting evidence.

Assessment reports must be sent to Opportunity Training within 14 days of the assessment.

Any additional evidence necessary shall be forwarded direct to Opportunity Training.

Assessors may, at their discretion, accept a previous assessment report as evidence of partial competency when assessing a trainee during a re-assessment.

Ultimately the trainees must be able to demonstrate competency in the performance criteria detailed in the unit standards to the satisfaction of the assessor.

Payment for assessment services shall be as agreed by the assessor and those contracting the service. On request Opportunity Training shall recommend an appropriate rate.
